How much does Davis Vision Insurance Cost?

Full-time employees are eligible to purchase vision coverage through Davis Vision. Employees electing coverage will pay monthly premiums as a deduction from each paycheck.

You Only Coverage - $5.00 per month
You + Spouse Coverage - $8.99 per month
You + Children Coverage - $8.49 per month
You + Family Coverage - $14.99 per month

Davis Vision is owned by Versant Health (a MetLife company). 
Some providers will be more familiar with the name Versant Health instead of Davis Vision.

Setup your Davis Vision Member Portal!

At www.davisvision.com, click on the red "Member Portal" button in the upper right corner of the screen to setup or log in to your member portal.  When registering, use your SSN for the ID#.  If that doesn't work, you can call Davis (1-800-999-5431) and ask them to give you your ID#.

Where is my Davis Vision Insurance Card?

Our Davis Vision Insurance does not require that you have a card to get coverage! If your provider is in-network with Davis Vision, simply tell them that you have coverage with Davis Vision, provide your full name and (the employee's) SS#, and the provider will be able to verify with Davis. Pretty simple!

Deciding between glasses or contacts?

Each enrollee is eligible for either glasses lenses OR contacts every 12 months (from the time of last purchase through Davis). Eligibility for glasses frames is every 24 months. Employees are encouraged to review the policy coverage for glasses vs. contacts in order to determine best value considering your personal situation. 

How do I get reimbursed for Out-Of-Network care or purchases?

Our Out-Of-Network benefits are as follows:

• Eye Exam: maximum reimbursement of $40
• Contacts: Elective: $105, Medically Necessary: $225
• Lenses for glasses
  • Single Vision: $40
  • Bifocal/Progressive: $60
  • Trifocal: $80
  • Lenticular: $100
• Frames: $50

If your Out-Of-Network claim was denied by Davis Vision - call them at 1-800-999-5431 to find out why.  Claims often get denied because the data entered on the forms does not match the data in their system exactly...such as putting Dr. on the form but the system has Drive spelled out!

Can I use my EBC HRA to pay for vision care?

Yes! You can use your HRA to pay for your costs associated with the eye exam and then you can use your HRA to pay for up to $100 per person for your costs for purchasing contacts to glasses. 

For example, if you have Davis Vision Insurance:

• You and your spouse get eye exams and must pay the two $10 In-Network copays.  
  • Use your EBC HRA card to pay the $20.
• You get contacts and after the $130 Davis Vision allowance is deducted, you owe $150 more.  
  • Use you EBC HRA card to pay for $100 and your personal card to pay for $50.
• Your spouse gets glasses (frames and lenses) and after the $130 Davis Vision allowance is deducted, you owe the $25 copay for the lenses and $50 more for the frames.  
  • Use your EBC HRA card to pay the $75.
• Total HRA funds used: $195
• Total paid out of your pocket = $50

Same example, if you do NOT have Davis Vision Insurance:

• You and your spouse get eye exams and must pay the two $120 exam fees.  
  • Use your EBC HRA card to pay the $240.
• You get contacts and owe $280.  
  • Use you EBC HRA card to pay for $100 and your personal card to pay for $180.
• Your spouse gets glasses (frames and lenses) and you owe $150 for the lenses and $180 for the frames.  
  • Use your EBC HRA card to pay for $100 and your personal card to pay for $230.
• Total HRA funds used: $440
• Total paid out of your pocket: $410

Don't forget to save the receipt in case EBC requests documentation to substantiate your charges!
