A special note about Medical Equipment:

Reimbursement for medical equipment from CHM is tricky so it’s always best to just call and ask about your specific situation.  Even though the Guidelines state that only equipment that is "necessary to sustain life" is eligible for sharing, often equipment that is "necessary for healing” is eligible for sharing.  A great example of this is a helmet for helmet therapy to correct a baby's skull shape.

CHM Member Portal:

To register for your CHM online member portal:

  1. Go to : https://chministries.org/members/ and click "Member Portal"
  2. Click "Register"

  3. 4. Enter the information asked and click "Sign Up". If you need your "Member #", ask Denise for it!


What can you do on the portal?

  1. Click the Envelope to see any notifications that CHM has sent you regarding documents you submitted.
  2. Membership Information: Update your address, email address, and phone number, Add a person to your membership, and Change your password.
  3. Submit Medical Bills
  4. Resources: View CHM's Guidelines
  5. Contact CHM: Send CHM a note using their internal email system.
